Change of Student Demographic Data Form

Complete this form to update your legal name, citizenship status, date of birth, and/or gender/gender identity. Students on Rutgers F-1 or J-1 visa sponsorship should reach out to the appropriate campus office listed below before submitting this form for any demographic data change. The change must be aligned with the information on your passport, I-20/DS-2019 and other legal documents.

Directory Information Exclusion Complete this form to restrict the information others can view about your student status.
Duplicate Diploma Request To request an additional diploma, complete this form. If the original diploma name is different from your current name, and you want the duplicate diploma to reflect your current name, you must also submit a Change of Student Demographic Data Form.
In-State Residency Analysis The purpose of this form is to provide the admissions office/office of the registrar with information to make a determination regarding your residency status.
Non Resident/Out of State Tuition Exemption Form Complete this form to request an exemption from paying non-resident tuition rates.
Voter Registration Form Complete this form to register to vote in local, state, and national elections.
Withdrawal Form Students wishing to cancel their registration, withdraw from all of their courses, or take a leave of absence from the university, must follow the University Withdrawal Process to submit an official online withdrawal form.
